Cashless Health Claim Settlement Checklist

For planned hospitalizations, notify the TPA desk or Aadhiraksha at least 72 hours before admission. For emergency admissions, inform the desk within 24 hours of hospitalisation.

Make sure to submit a copy of your Aadhiraksha health card and ID card (Aadhaar or PAN) to the pre-auth hospital desk immediately to avoid initial deposit payments.

Mandatory Claim Documents

Original discharge summary sheet from hospital.
Itemized billing invoices with receipt break-ups.
All original lab test results and radiology scans.
FIR Copy & Driving License (Mandatory for motor accidents).
